The Outreach Program goals are to identify and develop a relationship with single individuals, eighteen and over , who are chronically homeless according to HUD definition and assist them in obtaining housing  and other supportive services, as needed. The individuals served by this program must have a diagnosed physical or mental health disability.


The goal of the Housing program is to ensure that individuals secure and remain in stable permanent housing for at least two years. Typically the individuals of the Housing program fit the criteria for the outreach program and receive services to stay housed.


The goal of the Community group programs are to help clients meet their own goals as well as to help them integrate or re-integrate into the mainstream community.
